FANCY FEET DANCE
FALL FESTIVAL PARTICIPATION WAIVER

By registering for or participating in the Fancy Feet Fall Festival, I acknowledge and agree to the following:

ASSUMPTION OF RISK

I understand that the Fancy Feet Fall Festival may include dancing, games, crafts, outdoor activities, and inflatable attractions, including bounce houses, slides, and/or obstacle courses.

I understand that these activities involve inherent risks, including falls, slips, trips, collisions, sprains, strains, bruises, cuts, and other injuries. I understand that inflatable activities may involve additional risks, including falling, jumping, sliding, collisions with other participants, and other equipment-related injuries.

I voluntarily choose to participate, or allow my child to participate, in these activities.

SAFETY RULES

I agree that I and/or my child will:

Follow all posted rules and safety instructions.
Follow instructions from Fancy Feet Dance staff and inflatable attendants.
Follow all age, height, weight, and other requirements for inflatable equipment.
Use equipment appropriately and safely.
Immediately stop participating when instructed by event personnel.

Fancy Feet Dance reserves the right to remove a participant from an activity or the Event if safety rules are not followed or if the participant's conduct creates a safety concern.

MINOR PARTICIPANTS

If the participant is under 18 years of age, I confirm that I am the participant's parent or legal guardian and that I have authority to agree to this waiver on the participant's behalf.

I understand that parents/guardians are responsible for supervising their children during the event unless Fancy Feet Dance expressly provides otherwise.

RELEASE OF LIABILITY

To the fullest extent permitted by California law, I agree to release and hold harmless Fancy Feet Dance, its owners, employees, instructors, volunteers, contractors, event personnel, and affiliated parties from claims arising out of my or my child's participation in the Fancy Feet Fall Festival, including participation in inflatable activities, except for claims that cannot legally be waived or released under California law.

MEDICAL EMERGENCY

In the event of an emergency, I authorize Fancy Feet Dance or designated event personnel to seek reasonable emergency medical assistance for me or my child when reasonably necessary and when I cannot be reached.

I understand that I am responsible for any resulting medical expenses.

PERSONAL PROPERTY

I understand that Fancy Feet Dance is not responsible for lost, stolen, or damaged personal belongings brought to the event.

PHOTO & VIDEO

I understand that photographs and videos may be taken during the Fancy Feet Fall Festival.

Photo/Video Consent:
☐ I give Fancy Feet Dance permission to use photographs and/or video of me/my child for legitimate business, promotional, website, social media, advertising, or educational purposes.

☐ I do not give permission for photographs or video of me/my child to be used for these purposes.
