## LIABILITY WAIVER AND RELEASE
I, the undersigned parent or legal guardian of the minor participant named above, understand that participation in dance classes, rehearsals, performances, camps, workshops, events, and other activities provided by **Fancy Feet Dance** involves physical activity and may include risks of injury.
I understand that dance activities may involve stretching, jumping, turning, floor exercises, choreography, movement, use of dance equipment, and interaction with instructors and other participants. These activities may result in injuries including, but not limited to, sprains, strains, bruises, falls, or other injuries.
I voluntarily choose to allow my child to participate in activities provided by Fancy Feet Dance and understand the inherent risks associated with participation.
To the fullest extent permitted by California law, I agree to release and hold harmless Fancy Feet Dance, its owners, instructors, employees, independent contractors, volunteers, agents, and representatives from claims arising out of my child's participation in Fancy Feet Dance activities, except to the extent such release is prohibited by law.
I understand that this waiver does not eliminate or waive any rights or claims that cannot legally be waived under California law.
I confirm that I am the parent or legal guardian of the participant named above and have authority to sign this agreement on the participant's behalf.

## PHOTO AND VIDEO RELEASE
I give **Fancy Feet Dance** permission to photograph and/or record my child during dance classes, rehearsals, performances, camps, events, and other Fancy Feet Dance activities.
I understand that photographs and videos may be used by Fancy Feet Dance for legitimate business and promotional purposes, including:
* Fancy Feet Dance's website
* Social media accounts
* Advertising and promotional materials
* Flyers, brochures, and posters
* Email newsletters
* Recital and event promotions
* Studio displays and marketing materials
I understand that my child's name does not need to be included with photographs or videos.
I grant Fancy Feet Dance permission to use these photographs and/or videos without compensation to me or my child.
