### 1. PARTICIPATION

I understand that participation in dance classes, rehearsals, performances, camps, workshops, events, and other activities offered by **Fancy Feet Dance** involves physical activity and may involve risks of injury.

Activities may include, but are not limited to, ballet, tap, jazz, hip hop, lyrical, contemporary, musical theater, stretching, conditioning, choreography, jumping, turning, floor work, and other dance-related activities.

### 2. ASSUMPTION OF RISK

I voluntarily choose to participate, or permit my child to participate, in activities at Fancy Feet Dance.

I understand that risks may include, but are not limited to:

* Sprains, strains, and muscle injuries
* Falls and collisions
* Cuts, bruises, or other accidental injuries
* Overuse or repetitive-motion injuries
* Injuries resulting from physical activity or equipment
* Injuries resulting from the actions of other participants
* Injuries resulting from conditions of the studio, performance venue, or activity location
* Other risks ordinarily associated with dance and physical activity

I understand that no activity can be guaranteed to be completely free from risk.

### 3. HEALTH & FITNESS

I represent that the participant is physically capable of participating in the selected activities, or that I have obtained appropriate medical clearance to participate.

I agree to notify Fancy Feet Dance of any medical condition, injury, allergy, physical limitation, or other circumstance that may affect the participant's ability to safely participate.

Fancy Feet Dance and its instructors are not responsible for determining whether a participant is medically fit to participate.

### 4. RELEASE AND WAIVER

To the fullest extent permitted by applicable law, I agree to release and hold harmless **Fancy Feet Dance**, its owners, officers, employees, instructors, contractors, volunteers, agents, and representatives from claims arising from the participant's participation in Fancy Feet Dance activities, including claims for personal injury, illness, property damage, or other loss, except to the extent such release is prohibited by law.

This agreement is intended to apply only to the extent permitted under California law.

### 5. MEDICAL EMERGENCY

In the event of an injury or medical emergency, I authorize Fancy Feet Dance personnel to take reasonable steps to obtain emergency medical assistance for the participant when I or another authorized parent/guardian cannot be reached promptly.

I understand that Fancy Feet Dance is not responsible for medical expenses or treatment provided by emergency personnel, physicians, hospitals, or other medical providers.

### 6. PARENT/GUARDIAN CONSENT FOR MINORS

If the participant is under 18 years of age, I represent that I am the participant's parent or legal guardian and have authority to enter into this agreement on the participant's behalf.

I have read this agreement and voluntarily consent to the participant's participation in Fancy Feet Dance activities.

### 7. PERSONAL PROPERTY

I understand that Fancy Feet Dance is not responsible for lost, stolen, or damaged personal belongings brought to the studio or to events.

### 8. ACKNOWLEDGMENT

I acknowledge that I have carefully read this Liability Waiver, Release & Participation Agreement and understand its contents.

I understand that I am giving up certain legal rights by signing this agreement, to the extent permitted by law.

I have had the opportunity to ask questions before signing.
