Assumption of Risk

I, the undersigned parent or legal guardian, understand that participation in Fancy Feet Dance Holiday Dance Camp includes physical activities such as dance, games, crafts, movement activities, glow parties, and other recreational events. I acknowledge that participation may involve risks including, but not limited to, slips, falls, collisions, muscle strains, bruises, or other injuries.

I voluntarily allow my child to participate in all camp activities and assume all risks associated with participation.

Release of Liability

I release, waive, and forever discharge Fancy Feet Dance, its owners, directors, instructors, employees, volunteers, assistants, and representatives from any and all claims, liabilities, demands, actions, damages, or causes of action arising from or related to my child's participation in Holiday Dance Camp, except in cases of gross negligence or willful misconduct.

Medical Authorization

In the event of an emergency, I authorize Fancy Feet Dance staff to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that I am responsible for any medical expenses incurred.

Illness Policy

I agree not to bring my child to camp if they have a fever, vomiting, diarrhea, or any contagious illness. If my child becomes ill during camp, I understand that I will be contacted and must arrange prompt pickup.

Potty Training Requirement

I understand that all campers must be fully potty trained. Fancy Feet Dance staff are not permitted to assist children with toileting, changing diapers, or changing pull-ups.

Photo & Video Release

I grant permission for Fancy Feet Dance to photograph or record my child during camp activities and to use these images or videos for promotional, advertising, website, and social media purposes without compensation.

Personal Belongings

Fancy Feet Dance is not responsible for lost, stolen, or damaged personal belongings brought to camp.
